On 25 May 2017 the former Independent Commissioner Against Corruption announced an investigation into possible maladministration at the Oakden Older Persons Mental Health Service. The report from that review was released in February 2018.

The Office of the Chief Psychiatrist was asked to lead and coordinate the response to the report on behalf of the Department for Health and Wellbeing.

In February 2018 a Preliminary Report was released which outlined the work undertaken to that date. Subsequent to the preliminary report accompanying reports were also released that outlined the review of the Community Visitor Scheme undertaken by Julian Gardner and work undertaken by the Allied Health Office on Allied Health Professional Staffing in Mental Health Services.

In May 2020 a Final Report was released which detailed the further work undertaken since the release of the Preliminary Report into implementing the recommendations from the Oakden Report of the Independent Commissioner Against Corruption.
